Everything we need from you can be completed before you arrive. It takes about ten minutes in the comfort of your home, versus twenty minutes in a waiting room writting on a clipboard.

Download, fill them in at home, and bring them with you. Take your time and check your medication list against the bottles in your cabinet — that is the part everyone guesses at.
Everyone, every year
- Patient registration — name, contact details, emergency contact, pharmacy
- Medical and eye history — conditions, surgeries, medications, allergies, eye history
- Privacy policy — Privacy and HIPAA Acknowledgement
Check your patient portal.
Open the portal on a phone, tabletm or computer. You can see your most recent exams, testing, and medications.
First time here? You can only access the portal after your first appointment. Upon request, you will receive an invitation to set up your account. If you cannot find it, call the office and we will resend it.
What to bring every visit:
- Your insurance card and a photo ID
- Your glasses, and your contact lens boxes if you wear them
- A list of your medications, or the bottles themselves
- Sunglasses — if we dilate you, you will want them for the drive home
- Anyone who helps you keep track of things
